- How to complete a Lifestyle Questionnaire that identifies your personal life requirements
- How to complete a 'Self Assessment' for the Local Authority that you live in - this is required if you need Social Services support to pay for, or organise, your support
- How to seek help from Social Services to assess your needs with you
- How to seek help from elsewhere to assess your needs with you - if you do not want, or are not eligible, for Social Services support
