You may not know what services are available, let alone how to contact them. Help and Care’s Hospital Screening Service offers patients at Royal Bournemouth Hospital and Dorset County Hospital everything they need to make their return home as simple and stress-free as possible.

Preparing for your stay in hospital
If you are very unwell then you may need hospital care. This isn’t always, but can be, a daunting experience. If possible it is important to discuss your hospital admission with your GP as fully as possible in order to make sure you really understand what the experience will be like. We recommend that you discuss what will happen when you return home even before you are admitted. We think it is important for you, and for your GP, to know who might need to help you or what arrangements might need to be made.
Getting the right help when leaving hospital
Our team can visit you during your stay to ensure that you are as prepared as possible for the next stage – leaving. During that time, we can provide you with information on all of the services that may be available to you.
These include:
- Support and information for you, your family and/or carer
- Help putting you in touch with the services you may need but do not know how to contact, such as social services
- Fact sheets about organisations and services that might be able to help you, such as shops that can deliver to your home or meal delivery services
- Support from a Help and Care volunteer to settle you back into your home
- Support from a Help and Care advocacy worker who will help to deal with difficult issues, such as housing or financial problems
- Help contacting carers who can support you in your home

Concerned about a relative or loved one who is leaving hospital?
If someone you know or care for is in hospital and needs the screening team, please contact a nurse on the relevant ward. They will refer your loved one to the team on your behalf. If you are still concerned then callus on 0845 4500 418 and we will help.
