We’re part of a joined-up team of professionals working in every community across Dorset. From social prescribers to wellbeing coaches and care coordinators, we’re here to have meaningful, personalised conversations – getting to the heart of what matters to you. 

We work alongside the NHS, local councils, and voluntary organisations to ensure support is accessible, flexible, and rooted in each neighbourhood. Whether it’s navigating care options, connecting with community resources, or improving wellbeing, you’re not alone. 

Health and Wellbeing Coaching

Life doesn’t always go to plan — and when things feel overwhelming, it helps to have someone in your corner. 

Our Health & Wellbeing Coaches offer personalised support to help you build confidence, learn new skills, and find your own way forward. Together, we focus on what matters to you and how to improve your wellbeing, one step at a time. 

Social Prescribing

Sometimes the best support is community. 

Our Social Prescribing Link Workers take the time to understand your situation and connect you to practical, emotional, or social support in your local area. From housing advice to hobby groups, we help you feel more engaged, more empowered, and less alone. 

Memory Support & Advice

Memory concerns can be worrying – but you’re not alone. 

Whether you’re living with a cognitive impairment, have a dementia diagnosis, or are just starting to notice changes, our Memory Support team is here to guide you. We work as part of a multi-disciplinary team to offer advice, tools, and tailored support for you and those around you. 

Mental Health & Wellbeing Support

Everyone deserves support with their mental health – early, compassionate, and without judgement. 

Access Wellbeing offers one-to-one support for anyone aged 18+.

Whether you’re feeling low, facing life challenges, or just need someone to talk to, we’re here. We can help with emotional wellbeing, as well as related issues like money worries, work stress, or housing. Support is available online or face-to-face in local hubs. 

HOPE – Help Overcoming Problems Effectively

Help & Care is the certified provider of the HOPE Programme in Dorset, offering friendly, practical courses that help adults build confidence, improve wellbeing, and take charge of their health.

HOPE is a free six-week self-management course for people living with long-term health conditions or caring responsibilities. Led by trained facilitators, sessions blend proven techniques with supportive peer discussions to help you build lasting tools for life.

Roots

At Roots, we believe nurturing nature nurtures us too.

Our small team supports people living with mental ill health by providing gardening services for those who need a hand, and by tending to community green spaces across Bournemouth and Poole.

Volunteers are often also on their own recovery journey. Roots offers a safe, welcoming space to grow confidence, skills, and social connection — no gardening experience needed.

Healthwatch

Healthwatch is here to make sure health and social care services hear the voices of the people who use them.
