Sometimes, the things that impact our wellbeing aren’t medical at all – they’re social, emotional, or practical. That’s where our Social Prescribing Link Workers come in. 

They’re here to help you look at the bigger picture of your life – and support you to find community-based solutions that build resilience, reduce isolation, and empower you to move forward. 

How Social Prescribing Can Help

Your Link Worker will: 

  • Take time to understand you – not just your situation 
  • Help you overcome barriers that may be holding you back 
  • Support you in creating a plan to boost your health, wellbeing, and support network 
  • Connect you to local services, groups, and activities that suit your needs and interests 
  • Be there as a guide, cheerleader, and problem-solver along the way 

Whether it’s housing worries, loneliness, anxiety, or something else entirely – you don’t have to face it alone. 

What to Expect: 

  • Support takes place over a 12-week period 
  • Weekly or fortnightly sessions, tailored to your pace 
  • Sessions can be face-to-face, by phone, or via video call 
  • We’ll aim to book everything in advance, so you have a clear, manageable plan 

Our Link Workers are all about building trust and offering real, personalised support. No ticking boxes – just genuine conversations that lead to positive change.

Group programme sessions

HOPE

Help & Care is the certified provider of the HOPE Programme in Dorset, offering friendly, practical courses that help adults build confidence, improve wellbeing, and take charge of their health.

HOPE is a free six-week self-management course designed for people aged 18 or over who are registered with a GP in Dorset and ready to explore new ways of looking after their health. It’s ideal for anyone living with long-term conditions or facing the pressures of caring.

Community-led projects

Roots

At Roots, we believe nurturing nature nurtures us too.

Our small team supports people living with mental ill health by providing gardening services for those who need a hand, and by tending to community green spaces across Bournemouth and Poole.

Volunteers, often referred through mental health services, are also on their own recovery journey. Roots offers a safe, welcoming space to grow confidence, skills, and social connection — no gardening experience needed.
